A 41 year old, previously healthy patient, presented to our institution with acute penile pain and oedema, frank haematuria, dysuria, vomiting and fevers. Blood cultures isolated Peptinophilus species and Streptococcus anginosus. Dual imaging modalities demonstrated diffuse signal abnormality confined to the corpus spongiosum suggestive of deep soft tissue infection. Interrogation of possible sources led to an incidental finding of severe periodontal disease requiring extensive dental clearance. These findings, in the setting of polymicrobial bloodstream infection with typical oral flora, led to the diagnosis of spontaneous corpus spongiositis secondary to haematogenous seeding from periodontal disease.
